Check List to apply for wholesale license for the state of Louisiana

 

  1. If you are using a business name to advertise your business then you are required to regiser it as a sole proprietor, corporation, llc, partnership, or check your accountant or attorney to get the best business structure for your business.
  2. It is valid for one year.
  3. You are required for you to submit the approved copy of your business name registration with the secratary of state or the county clerk office.
  4. Provide proof of the bond(s) required by R.S. 47:848A and/or 849C on file with the excise tax division of the Louisiana Department of Revenue. If stamping agent, provides proof of direct purchase of contract(s) with tobacco manufacturer(s), importer(s), or supplier(s).
  5. Provide a diagram and photographs applicant’s warehouse.
  6. Provide photographs of the stamping machinery to be used in stamping cigarettes (if applicable).
  7. Application must be signed by the owners/officers or by an authorized agent.
  8. If any person with an ownership in the business has been convicted of a felony, you must complete schedule F form
  9. To avoid delays in your application process, verify that you have fully completed the application packet and applicable schedules prior to submission.
